e.The applicant and the insureds declare and affirm that the applicant and the passengers (i.e. the insureds) agree on the enrollment and the sum insured, after they read all the insurance terms and accepted the provisions on such major issues as exclusions and surrender, and that they have provided authentic and accurate information for application and are willing to be liable for any concealment or false declaration.

(3)Insured Period:The duration of insurance in this insurance contract starts when the insured gets through the security check before boarding the one-way flight (or the alternative flight arranged by the carrier) covered by the insurance policy (or the air transport of electronic one-way tickets marked with the premium) and terminates when the insured arrives at the final destination by the flights mentioned above and gets to the airport secured area.

(4)Insurance coverage and features (Please refer to the above tables for the amount insured of “CSAir E-Travel Insurance 2015”)

a.Passenger accidental death and dismemberment

We will make a one-off payment of the insurance benefit in case of accidental death, injury or disability of the insured when the insured take the public transportation means arranged by CSA.

b.Passenger accidental injury medical costs

Medical costs caused by aviation accidents ,with the maximum amount of RMB50,000.

c.Flight delay

(1)If the flight that the insured plans to take is delayed by 3 hours or more due to bad weather, natural disasters, mechanical failure, strikes, hijacking or sabotage, temporary protests of other air, maritime workers, terrorist acts, air traffic control or airline overbooking, and the insured does take the flight to the destination, he/she can make a claim.

If the flight that the insured plans to take is canceled due to any of the above reasons and the insured is delayed for three hours or more consecutively, the insured can make a claim, provided that the insured takes an alternative flight arranged by CSAir to the destination.

(2) Delay time is determined by the following time, whichever is longer: (1) from the scheduled departure time to the actual time of departure, or to the departure time of alternative transportation arranged by the airline; or (2) from the scheduled arrival time to the actual time of arrival time, or to the arrival time of alternative transportation arranged by the airline.

(3)Compensation standard: RMB300 for 3-hour delay, RMB100 for additional one-hour delay, the maximum compensation being RMB600.

No compensation if the delay is less than 3 hours; the delay is counted at 3 hours if the actual delay exceeds 3 hours but is under 4 hours; the delay is counted at 4 hours if the actual delay exceeds 4 hours but is under 5 hours; the delay is counted at 5 hours if the actual delay exceeds 5 hours but is under 6 hours;

(4)1、If the insured does not take the original flight or its substitute flight, or claims refund, he/she is allowed to cancel the policy and no insurance payment will occure.

2、If the actual arrival time of the substitute flight arranged by the carrier is earlier than the scheduled arrival time of the original flight of the insured or the insured does not take the earliest substitute flight arranged by the carrier, then no insurance payment will occur.

3、This insurance policy won’t underwrite the flight delay or cancellation caused by the flight seasonal plan adjustment.

4、If the Insured willingly applies for more than one insurances which have the same coverage under different insurances (excluding the insurance of which the policy-holder is a group) underwritten by the Insurer for the same oversea working purpose, the Insurer will make the indemnity only under the policy that has the highest insured amount, and refund the corresponding insurance premiums collected under the same coverage for other insurances.

d.Luggage delay

During the Insurance Period, the Insurer will indemnify the Insured for the specified Insurance Amount when the consigned baggage of the Insured does not arrive within the period stipulated in the Policy after the Insured arrives at the destination due to the airline company who receives the consignment of the transportation of the baggage by the Insured during the travel and transports the baggage through the scheduled flight that the Insured takes.The delay time is calculated based on the time gap between the time when the insured arrives at the destination and the time when the insured sign for the baggage. Compensation standard:RMB1,000 for 6-hour delay, with the maximum amount of RMB1,000.

You need to submit the proof of “luggage delay” issued by CSAir or the airport, including the flight number and date as well as length of luggage delay.

e.Loss and damage of checked luggage

In case of damage and loss of the checked luggage occurring from the time of checking to the time of luggage claim, the insured can get the corresponding compensation, with the maximum amount of RMB600.

You need to submit Written documents issued by CSAir or the airport proving destruction, loss or damage of checked luggage and relevant proofs of indemnity.

f.Loss and damage of carry-on belongings

If the insured’s carry-on belongings are lost or damaged due to theft or robbery during the insured period, the insured can get compensation that can go up to RMB1,000.

g.Aircraft hijacking

Should the insured run into a hijacking incident during travel, the insured can get a compensation of RMB10,000.

(5)Exclusions:

a. The Insurer will not pay benefit if any of the following cause results in the Insured’s death, injury and disability or payment of medical expenses:

1. Intentional act of the Applicant;

2. The Insured’s committing self-hurt or suicide, except that the Insured is a person without capacity for civil conduct when committing suicide;

3. Fighting, being attacked or murdered caused by the insured’s provocation or intentional act;

4. The Insured’s pregnancy, miscarriage, childbirth, disease, drug allergies, heat stroke, sudden death;

5. The insured’s receipt of cosmetic surgery and other medical operation or surgery;

6. The insured’ taking, using, injecting drug without doctor's advice;

7. Nuclear radiation, nuclear explosion or nuclear pollution;

8. Terroristic attack;

9. The Insured commits crime or resists arrests;

10. The Insured suffers accident after passing through security check and then leaving the airport;

Article 6 If the Insured suffers death, injury and disability or medical expenses occurs during the following period, the Insurer shall not be liable to pay benefits:

1. War, military actions, riots or armed rebellion;

2. When the Insured is drunken or under the influence of drugs or controlled substance

Article 7 The Insurer shall not pay benefit for the following expenses:

1. The items and medicines at the Insured’s own expense as stipulated by social medical insurance or other public medical management authority where the policy is issued;

2. Medical expense caused by the Insured’s disc bulging or disc protrusion;

3. Nutrition fees, rehabilitation fees, auxiliary appliance expense, face-lifting fees, cosmetic fees, repairing operation fees, tooth cosmetic fees, tooth repairing fees, prosthodontics fees, attendance expenses, traffic expenses, accommodation fee, loss of income, funeral expenses.

Should the Insured die because of the occurrence of any situation of the above Article 5 and 6, the Insurer’s liability to the Insured hereunder shall be terminated, and the Insurer shall return to the Applicant the unearned net premium calculated on daily pro-rata basis (one day shall be calculated if the passed time is less than 24 hours).

The Insurer shall not be liable for the following circumstances:

1. Flight delay or cancellation is caused by the Insured;

2. The Insured has known or should have known through reasonable assumption that the delay may be equal to or longer than the time specified in the insurance policy when booking a flight or applying for an insurance.

3. Scheduled departure time is not falling within the period of insurance;

c. Luggage delay

This rider is not applicable to the exclusions under the main insurance, but the Insurer shall not be liable for any loss directly or indirectly caused by or arising from:

1. The consigned baggage is detained, confiscated, segregated, examined or destroyed by the custom or other governmental organizations;

2. War, civil warfare, military actions, terrorist activity, strike, riot, or turmoil, or nuclear explosion, radiation or pollution.

d.Luggage loss

1. Loss caused by the Insured’s failure to comply with national laws, government regulations, orders and requirements;

2. Destruction of, loss of or damage to personal effects carried by the Insured;

3. destruction of, loss of or damage to consigned luggage completely caused by nature, quality or defect of such luggage;

4. Loss caused by natural disaster or other out-of-control causes (war or military conflict);

5. Shortage of or damage to the internals with complete packing and seal, unless it is evidenced that such shortage or damage is caused by the airline due to its negligence, the Insurer shall not be liable for loss of luggage;

6. The Insured does not raise an objection when picking up the luggage and the carrier does not provide Error Records of Luggage Transportation and Damage Records of Luggage;

7. Indirect loss caused by loss of luggage;

8. Consigned luggage hanging a Non-responsibility Card;

9. The Insurer shall not be liable for the part of overweight for which is not paid;

10. Other items which can not be put into the luggage according to the relevant requirements, such as fragile items, perishable goods, valuables, documents, certificates, securities, cash, gold and silver jewelry, for which the Insurer is not liable;

11. Loss caused by the Insured’s negligence;

12. Loss caused by delay on consigned luggage.

e. Loss and damage of carry-on belongings

1. Gold, silver, jades, jewelry, crustal and its product, glass and its product, cash, securities, bills, credit card, token, document, animal, plant, food and drink, furniture, antique and curio;

2. Data recorded on the tape, dish, disk or similar carrier, excluding the material cost of the carrier;

3. The loss of commercial good or sample;

4. Mobile telephone ( or cell phone), portable computer, PDA;

5. Direct or indirect loss due to governmental or judicial actions, including confiscation, detainment by the Custom;

6. Scratch and/or dent that do not influence use;

7. Loss without reason or mysterious disappearance;

8. Loss of the motor vehicle and its accessory, ship, airplane or other conveyance;

9. Consequential loss;

10. For each item whose loss exceeds RMB 1000 Yuan, the Insured can not provide the original invoice.

(6)Age limit:The entry age of the insured must be within the scope from 14 days to 90 years old. For the insured aged from 71 years old to 90 years old, the insured amount shall be halved while the premium remains unchanged.

(7)Insurance coverage limit:Each customer is limited to one purchase of insurance while the excessive part shall be void。

(8)Premium payment: Passengers applying for this product shall pay the premium and the air fare at the same time.

(9)Insurance contract: Passengers choose to purchase this product while booking a plane ticket and payment of the premium shall constitute the signing of the insurance contract.

(10)Insurance policy:The E-travel insurance adopts the innovative electronic insurance sales model. As a real paperless insurance product, it does not provide any paper insurance document. You can check and confirm your insurance policies by visiting the insurance company’s website or calling its customer service.

(11)Insurance invoice: Insurance combines the premium invoice with the E-ticket, and the amount of premium is shown in the column of “premium” in the E-ticket. If the insurance is not shown on your flight itinerary, you may contact China Southern Airlines customer service for more information.

(7)Can I cancel my CSAir E-Travel Insurance? How do you cancel it?

If you cancel the flight, the insurance premium will be automatically returned with the ticket refund. You can also choose to just cancel the insurance before the scheduled departure time. (8)If I change the CSAir flight, will the E-Travel Insurance I purchased still be valid?

Whether you change the CSAir flight voluntarily or involuntarily, as long as the actual carrier is still CSAir, your E-Travel Insurance will still be valid. If you change to a flight whose actual carrier is not CSAir, your E-Travel Insurance cannot be transferred to the new flight. Then you need to call our service hotline 40066-95539 to cancel the insurance in accordance with the insurance cancellation rules. The CSAir e-commerce counter will nor handle cancellation of insurance.

To apply for insurance claims log onto (https://b2c.csair.com/B2C40/modules/services/insurance/apply.html). b.The means of public transportation:The means of public transportation covered by the insurance policy refer to the fixed wing aircraft operated by the airlines agreed between the insurer and insured, the airport shuttle buses and other buses arranged by the airlines agreed between the insurer and insured.

c.Any insurance accidents happening on the direct flights bound for Afghanistan, Cuba, Democratic Republic of Congo, Iraq, Liberia, the Sudan and Syria or those happening during the travel in the countries mentioned above won’t be covered by this insurance policy.

d.Any terrorists or terrorist organization members recognized by any states or international organizations and traders illegally dealing with drugs, nuclear weapons and biological or chemical weapons won’t be covered by this insurance policy.

e.The Insurer shall not be liable for the scheduled flight is canceled or delayed 12 hours before the departure time.

f.How does the insured person provide an account of a minor?

Minors are covered by their guardian. Please select "other" in the China Southern Airlines self-service claim page where you fill in the bank account name and the passenger name and upload the guardian relationship certificate according to the prompts. If you use a household residence booklet, then provide the main page, page of the insured person and the guardian page. If you provide an ID card, then please also provide a child's birth certificate and both sides of the guardian's ID card.

g.What do I do if I am unable to submit the self-service insurance claim?

4.Insurance Broker Commission Contract And Customer Notification

Insurance Broker Commission Contract

Thanks to the customer (hereinafter referred to as “Party A”), China Southern Airlines Insurance Brokers Co., Ltd. (hereinafter referred to as “Party B”) is entrusted to handle insurance business. This contract is a valid agreement between Party A and Party B. Party B shall provide insurance brokerage services to Party A in accordance with the provisions of this contract, and Party A shall enjoy the rights and obligations of this contract.

The terms of this contract have a significant relationship with Party A's rights and interests. Party A is requested to confirm that Party A has fully read, understood and accepted all the contents of this contract before agreeing to accept insurance brokerage services (hereinafter referred to as "this service"). If Party A checks/clicks "Read and agree", "Agree to the terms of the contract" or otherwise chooses to accept this contract through the insurance order page, it means that Party A fully agrees and accepts all the terms of this contract.

Article 1 Party A entrusts Party B as an insurance broker to represent or assist Party A in handling insurance related matters.
